Its four crucial violations were its food handler not washing hands as necessary to prevent contamination of food and failing to protect food from contamination or adulteration.
The eatery also stored ice in an unsanitary manner and maintained potentially hazardous foods at the proper temperatures.
Of the nine significant infractions Pho Anh Vu, racked up, three pertained to its utensils. Specifically, using utensils not of readily cleanable material, utensils that are not free from crevices in contact with food, nor were they sanitized in the proper chlorine solution.
The restaurant failed to maintain its hand-washing stations with soap and paper towels, provide a thermometer in its refrigeration equipment or use food equipment not of sound and tight construction.

The mechanical equipment was also not maintained to provide the sufficient chemical solution rinse.
The facility’s surfaces were not cleaned as necessary, and a dirty cloth was used to clean surfaces that come into contact with food.
Four of the five minor infractions Pho Anh Vu received had to do with the food-handling room, including it not being free from equipment not regularly used, its floors, walls and ceilings were not maintained.
The establishment also failed to ensure that equipment surfaces were sanitized as necessary.
